Zusammenfassung
Die Pharmakotherapie mit Glukokortikoiden wird nahezu in allen Gebieten der Medizin praktiziert, aber auch nach vierzigjähriger Anwendung weiterhin kontrovers diskutiert. Für die sichere Beherrschung dieser Therapieform ist es wichtig, sich deutlich zu machen, daß es sich bei den Glukokortikoiden um ein ubiquitäres Regulationssystem handelt, das spezifische Effekte auf den Kohlenhydrat-, den Lipid-, den Protein- und den Nukleinsäurestoffwechsel ausübt. Dabei gibt es keinerlei Veranlassung, die häufig bezüglich ihres primären molekularen Wirkmechanismus noch gebräuchliche Unterteilung in physiologische und pharmakologische Glukokortikoideffekte aufrecht zu erhalten. Die Einflußnahme der Glukokortikoide, nicht zuletzt auf das Immunsystem des menschlichen Organismus, ist derartig vielgestaltig, daß schon vor längerer Zeit der Versuch unternommen wurde, diese Vielfalt der Glukokortikoidwirkungen durch einen zentralen Überbegriff zu beschreiben. Der bekannteste diesbezügliche Versuch ist die Formulierung von Hans Selye, der 1946 die Glukokortikoideffekte mit dem Begriff „Increased Resistance to Stress“ umschrieb [1].
