Hypertriglyceridaemia (HTG) as independent risk factor for myocardial infarction in type 2 diabetes: The Diabetes Intervention Study

Abstract

The Diabetes Intervention Study is a prospective controlled three-part-study with 1 139 newly detected NIDDM. The present report analyses the significance of hypertriglyceridaemia (HTG) as an independent risk factor for myocardial infarction in NIDDM. Furthermore we tested whether “good” TG-control according to the target levels of the NIDDM-Policy Group reduces the incidence of infarction. In multivariate analysis TG was shown to be an independent risk factor ranking second after blood pressure. The incidence of myocardial infarction in the subgroup with TG < 1.7 mM was 32 % and with TG > 2.3 mM 106 % (p > 0.01) — our data confirm HTG as an independent risk factor in NIDDM. Strict control of TG-level was associated with lower incidence of infarction.

Zusammenfassung

Die Diabetesinterventionsstudie (DIS) ist eine prospektive dreiarmige kontrollierte Studie mit 1139 repräsentativen neu diagnostizierten Typ-II-Diabetikern. In der vorliegenden Arbeit wurde analysiert, welchen Stellenwert die Hypertriglyzeridämie (HTG) als Risikofaktor für den Infarkt bei dieser Krankheitsgruppe hat und inwieweit durch optimale TG-Einstellung entsprechend den sogenannten ‚Euronormen‘ die Infarktinzidenzgesenktwerden kann. Bei multivariater Auswertung waren die TG nach der Hypertonie der wichtigste unabhängige Risikofaktor. Bei TG-Werten < 1,7 mmol/I betrug die Infarktinzidenz 32 %, bei TG-Konzentrationen > 2,3 mmol/I 106 % (p < 0,01). Die Ergebnisse zeigen, daß die HTG bei NIDDM (nichtinsulinabhängigen Diabetikern) ein unabhängiger Risikofaktor ist. Konsequente Korrektur der HTG reduziert das Infarktrisiko.
