Antikonvulsiva und Lithium als Adjuvantien der medikamentösen Therapie schizophrener Psychosen

Kombination therapeutischer Strategien bei schizophrenen Erkrankungen

Zusammenfassung

Seit ihrer Einführung im Jahre 1952 (Delay et al., 1952) stellen Neuroleptika die Therapeutika der Wahl zur Akut- und Langzeitbehandlung schizophrener Psychosen dar. Die in zahlreichen kontrollierten Studien erwiesene Wirksamkeit der Neuroleptikatherapie unterliegt jedoch einigen Begrenzungen:

  1. 1.

    Es gibt neben 10–20% »Placeborespondern« eine Gruppe von 30–40% »Nonrespondern«, die trotz neuroleptischer Medikation nicht ausreichend remittieren oder einen Rückfall erleiden.

  2. 2.

    Die Therapie mit Neuroleptika ist mit störenden Nebenwirkungen behaftet, die von groben extrapyramidalen Symptomen, Akathisie, über depressiv-apathische Syndrome bis zur Spätdyskinesie reichen und trotz guter antipsychotischer Wirksamkeit die sozialen Fähigkeiten erheblich einschränken können.
