Zusammenfassung
Dem Arzt im Krankenhaus sind vielfältige biomedizinische, psychologische und soziale Aufgaben gestellt. Zusätzlich zur naturwissenschaftlich begründeten Diagnostik und Therapie werden Fragen nach der Bewältigung, der Lebensqualität und sogar nach psychosozialen Einflüssen auf die Prognose bei schwer und chronisch Kranken bereits im klinischen Alltag relevant. Funktionelle Körperbeschwerden werden oft erst im Krankenhaus durch Ausschluß etwaiger organischer Leiden geklärt. Die vor allem von angelsächsischen Epidemiologen ermittelten Prävalenzraten sind nicht ganz eindeutig, wir können aber bei vorsichtiger Schätzung 30% psychogener Störungen bei den Patienten eines allgemeinen Krankenhauses erwarten (Lipowski 1979, Huyse 1989, Mayou und Hawton 1986).
