Zusammenfassung
Ein reifgeborener Junge wird in der 39 + 5 SSW komplikationslos per Spontanpartus entbunden. Bei der Erstinspektion fällt ein generalisiertes monomorphes Exanthem mit disseminiert verteilten Papulopusteln auf. Bei ansonsten klinisch völlig unbeeinträchtigtem Kind werden bis auf eine Laborentnahme und einen mikrobiologischen Abstrich einer eröffneten Pustel keine weiteren Maßnahmen getroffen. Am 2. Lebenstag zeigt sich ein deutlicher Anstieg der Entzündungswerte (CRP 6,8 mg/dl, IL-6 1193 pg/ml). Es wird eine empirische antibiotische Therapie begonnen, das Kind auf die neonatologische Intermediate Care Station verlegt und weitergehende Diagnostik initiiert. Die Mutter berichtet von einer zweimaligen vaginalen Mykose während der Schwangerschaft sowie eines klinisch inapparenten Herpes genitalis, woraufhin die Therapie des Kindes um Aciclovir und Fluconazol erweitert wird. Am 3. Lebenstag kommt es zu einem weiteren CRP-Anstieg auf 18,4 mg/dl, und der Junge entwickelt Fieber.
