Inlay Patellofemoral Arthroplasty

Abstract

Background: Isolated arthroplasty of the patellofemoral joint has been invented long ago. However, poor clinical outcomes and high failure rates have been reported in the past. Currently available trochlear implant designs can be divided into inlay and onlay designs. This chapter describes the technique and philosophy of a current inlay design and provides an overview of the available literature regarding the outcome.Results: Currently, there are some case series and only one trial comparing an inlay with an onlay design. Progression of tibiofemoral OA was the most common reason for failure of PFA using modern prosthetic designs. An interesting finding of the comparative study was that none of the patients with an inlay component showed progression of tibiofemoral osteoarthritis (OA), whereas more than half of the patients in the onlay group demonstrated progression of medial and/or lateral tibiofemoral OA.Conclusion: The hypothesis for this observation is the more anatomic approach of the inlay design that better reproduces the complex kinematics of the patellofemoral joint, by preventing soft tissue irritation due to overstuffing.
