Zusammenfassung
Grundsätzlich können valgisch impaktierte proximale Humerusfrakturen auch konservativ behandelt werden. Die radiologische Fehlstellung der Tubercula resultiert aus der Impaktion des Humeruskopfes. Dennoch haben wir uns bei diesem Patienten für eine Osteosynthese entschieden, insbesondere auch um den Kopf-Schaft-Winkel wieder herzustellen. Der Humeruskopf ist in etwa um 130° nach kranial relativ zum Schaft gewinkelt. Diese Neigung ist für die Rotatorenmanschette von Relevanz. Durch die Impaktion resultiert die relative Fehlstellung der Tubercula und dadurch wird die Kinematik der Rotatorenmanschette gestört. Durch den osteosynthetischen Aufwand ist es uns gelungen, den physiologischen Kopf-Schaft-Winkel wieder herzustellen.
