Zusammenfassung

Die Arthrose des Fingermittelgelenks (proximales Interphalangealgelenk, PIP) ist eine häufige Erkrankung der älteren Patienten und wurde bereits durch Charles Bouchard (Pathologe in Paris, 1837–1915) vor über 100 Jahren schriftlich erwähnt. Diese Gelenkdestruktionen können isoliert oder an mehreren Gelenken gleichzeitig (Fingerpolyarthrose) auftreten. Dagegen sind posttraumatische Gelenkzerstörungen oder entzündliche Erkrankungen (rheumatoide Arthritis, Psoriasis, Gichtarthropathie, etc.) deutlich seltener als die primäre Form der Fingergelenkarthrose. In diesem Kapitel stellen wir zwei Arten der Arthroplastik am proximalen Interphalangealgelenk vor. Die jeweiligen Zugangswege und die Operationstechniken werden im Detail erläutert und die einzelnen Operationsschritte bildlich dargestellt. Einerseits wird der dorsale zentrale Sehnensplit-Zugang zur Implantation der Oberflächengleitprothese CapFlex-PIP und andererseits der palmare Zugang zur Implantation eines Silikon-Spacers aufgezeigt.
