Zusammenfassung
Ein 12 jähriges Mädchen stellt sich primär beim Hausarzt mit einem zentralen Handgelenkschmerz, ohne Trauma in der Anamnese, vor. Unter der Diagnose einer Sehnenscheidenentzündung wird mit lokalen Salbenapplikationen sowie einer Schiene für ein paar Wochen therapiert. Wegen Persistenz der Beschwerden wird der Handchirurg aufgesucht, der in einem ersten Schritt, ohne spezielle weitere Abklärungen, eine Steroidinfiltration durchführt. Diese wirkt für mehrere Monate, dann kommt es aber zu einem Rezidiv der Beschwerden. Es wurde dann die Indikation zur Handgelenksarthroskopie gestellt und diese auch durchgeführt. Intraoperativ fand sich etwas Synovialitis radiocarpal und lockere Bandstrukturen, sowohl im Scapho-Lunären wie im Luno-Triquetralen Band. Aus diesem Grund wurde in derselben Sitzung ein thermisches shrinking dieser Bandstrukturen durchgeführt (◘ Abb. 14.1).
