Zusammenfassung
7 Monate alt gewordener Säugling mit Schädelfraktur rechts parietookzipital und epiduralem Hämatom. Das Kind habe einen Tag vor dem Tod mehrfach erbrochen, sei anschließend zum Schlafen ins Bett gelegt worden und abends leblos aufgefunden worden. Nach einer protrahierten Reanimation wird das Kind tief komatös in die Klinik aufgenommen und verstirbt einen Tag später. Vom Augenarzt werden Netzhautblutungen nachgewiesen. Die bei der neuropathologischen Sektion festgestellten Befunde mit filmartigem subduralem Hämatom, mikroskopisch kleinen Subarachnoidalblutungen, Hirnödem, flächenhaften Einblutungen in alle Schichten der Retina und in die Sehnervenscheide sowie der Nachweis einer lokalen traumatischen axonalen Schädigung im oberen Halsmark als Ausdruck einer Überdehnung des kraniozervikalen Übergangs sprechen aus neuropathologischer Sicht für ein stattgehabtes Schütteltrauma, wobei die Schädelfraktur mit epiduralem Hämatom als zusätzliches Anstoß-/Sturzgeschehen zu werten ist.
