Zusammenfassung
Die alles entscheidende Phase der Palliativversorgung bildet die Terminalphase. Gelingt die Begleitung, verliert der Tod oft seinen Schrecken. Ist aber nur eine Stunde leid- oder qualvoll, kann dies die liebevolle Umsorgung über Wochen Vergessen machen. In den meisten Fällen braucht es hierfür nur einfache, aber wirksame Strategien zur Leidenslinderung mit dem Fokus auf der Antizipation typischer Belastungen am Lebensende: Setzen neuer Schwerpunkte in der Begleitung, Vorsorgeplanung für typische Krisensituationen, Umsetzen der Medikation, Sicherstellen der Erreichbarkeit, enger Kontakt zum Team und die Mitbetreuung der Angehörigen sind wichtige Punkte. Besondere Bedeutung kommt Vigilanzstörungen am Lebensende zu, die fast immer auftreten. Hier helfen „Wohlfühlangebote“. Auch das Wissen um Nahtoderfahrungen, die im Rahmen palliativer Begleitung auch Sterbebettvisionen genannt werden, kann hilfreich sein. Palliativversorgung endet nicht mit dem Tod: Bereits ein gutes Gespräch nach der Todesfeststellung kann den Trauerprozess erleichtern.

Unter palliativer Chemotherapie wird ein nicht auf Heilung zielender Therapieansatz verstanden. Hauptziel ist eine Lebenszeitverlängerung, mithin ist der Name „palliativ“ zart missbräuchlich.

Anmerkung: Die weit verbreitete Auffassung, eine „Chemotherapie ausschließlich auf Wunsch des Patienten“ fortzusetzen oder durchzuführen oder auch eine „Chemotherapie zur psychischen Stabilisierung“ ist rechtlich und ethisch problematisch. Sie weist auf einen Kommunikationsmangel hin.
