Abstract
In early to moderate elbow arthritis, debridement procedures can improve pain and mobility and postpone advanced arthritis. The Outerbridge and Kashiwagi ulnohumeral arthroplasty involves a posterior incision and debridement of the posterior compartment. A fenestration of the olecranon fossa is performed to access and allow debridement of the coronoid process. Minimally invasive and arthroscopic versions of the technique have been described and are discussed in this chapter.
Elbow arthroscopy allows all compartments to be visualized and enables the surgeon to perform a more extensive anterior debridement. The arthroscopic OK procedure is increasingly being performed in the young and active patient with early degeneration, including the sportsmen.
The OK procedure is a reliable technique that improves pain, motion, and locking. The arthroscopic method facilitates a faster rehabilitation. Due to the risk of a supracondylar fracture, return to contact sports is delayed for 6 weeks.
