Abstract

Elbow anatomy is complex and can be challenging when treating degenerative and traumatic conditions. Not only the bony configuration of the distal humerus, the proximal radius, and the proximal ulna must be respected, but also the complex neurovascular anatomy must be well understood to prevent iatrogenic injury. The chapter on arthroscopic elbow anatomy will help navigating in the joint and to visualize the topography of the relevant periarticular structures.

The dorsal aspect of the elbow joint is confined by the olecranon fossa. On its medial border, the ulnar nerve is found. It is closely related to the medial articular capsule and by that is endangered during arthroscopic dissection in the medial gutter.

The posterolateral compartment contains the posterolateral plica, the posterior part of the radial head, the capitulum, and the ulno-humeral joint. Further on, the lateral ligament can be visualized.

Ventrally, the coronoid process, the ventral trochlea, and the ventral part of the radial head are accessible. It is important to appreciate the course of the median nerve ventrally to the trochlea and the course of the deep branch of the radial nerve en face of the medial border of the capitulum.
