Zusammenfassung
Bei älteren Patienten mit reduzierter Knochenqualität sind Insuffizienzfrakturen des Os sacrum relativ häufig und typischerweise mit starken invalidisierenden Schmerzen verbunden. Bisher gilt die konservative Therapie als Standard, wobei strikte Bettruhe und Schmerzmedikation sowie schmerzadaptierte Bewegungsmaßnahmen individuell angepasst werden. Patienten mit starken invalidisierenden Schmerzen lassen sich jedoch schlecht mobilisieren, entwickeln eine hohe Rate an Komplikationen und eine nicht zu akzeptierende Mortalitätsrate. Diese Patienten profitieren deutlich von einer Zementaugmentation mittels interventioneller, minimalinvasiver Behandlungsoption. Nachfolgend werden die CT-gesteuerte Ballonsakroplastie, Radiofrequenzsakroplastie, Vertebrosakroplastie und Zementsakroplastie bei nichtdislozierten Insuffizienzfrakturen erläutert und im Methodenvergleich hinsichtlich klinischer Verbesserung, Wirtschaftlichkeit und möglicher Komplikationen mit der neuesten Literatur diskutiert.
