Zusammenfassung

Bei der Zwerchfellhernie (CHD) handelt es sich um einen angeborenen Defekt des Zwerchfells, bei dem es, je nach Größe des Defektes, zu einer Verlagerung von Abdominalorganen (vor allem Magen, Darmschlingen und Leber) in den Brustkorb kommt. Es resultiert eine Verminderung des intrathorakalen Platzangebotes, wodurch das fetale Wachstum der mediastinalen Organe und vor allem der Lunge behindert wird. Pränatal bedingt die Lungenhypoplasie keine Hypoxie des Fetus, da die Sauerstoffversorgung des Kindes komplett über die Plazenta erfolgt. Erst postnatal ergeben sich für diese Kinder aufgrund ihrer veränderten kardiopulmonalen Situation erhebliche Probleme. Obwohl sich die pränatalen Diagnosemöglichkeiten in den letzten Jahren verbessert haben und erhebliche Fortschritte in der postnatalen Versorgung gemacht wurden, handelt es sich bei der CDH nach wie vor um ein lebensbedrohliches Krankheitsbild
