Zusammenfassung
Die Fournier-Gangrän ist eine rasch fortschreitende Infektion des Genital- und Perinealbereichs und stellt einen akuten urologischen Notfall dar. Es handelt sich hierbei um eine Sonderform der nekrotisierenden Fasziitis, die sich auf diese Regionen beschränkt.
Weiterhin können Trauma und Tumore ausgedehnte Skrotalschäden verursachen.
Der Anspruch nach einer definitiven Rekonstruktion in Bezug auf Ästhetik und Funktion ist hoch. Primärverschluss, Hauttransplantationen und lokale Fasziokutanlappen entsprechen in den meisten Fällen nicht diesen hohen Anforderungen. Die Autoren präsentieren die Rekonstruktion des Skrotums durch bilaterale gestielte Gracilis-Muskellappen und Spalthauttransplantate, eine Technik, die zu zufriedenstellenden, ästhetischen und funktionellen Ergebnissen führt.
