Zusammenfassung

Die DIEP und ms-TRAM-Lappenplastiken sind Standardlappenplastiken in der Eigengewebsrekonstruktion. Die ersten TRAM-Lappenplastiken, transverser Rectus-abdominis-Muskel-Lappen, wurden mit einer queren Hautinsel am M. rectus abdominis kranial gestielt in die Brust transponiert. Die Durchblutung kommt bei der Lappenplastik aus der A. epigastrica superior. Nachteilig ist, dass die quere Unterbauchhautinsel eigentlich dominant aus der A. epigastrica inferior versorgt wird, und die Anastomosen zwischen der superioren und inferioren epigastrischen Arterie nicht immer ausreichen, den Unterbauch komplett zu versorgen. Die Entnahme des gesamten Muskels und der Faszie führte mitunter zu Problemen der Bauchdecke, wie Bauchwandschwächen oder gar -hernien, konsekutiven Netzeinlagen oder Schmerzen.
