Zusammenfassung
Bei lokaler Irresektabilität eines malignen Lebertumors, dem Vorliegen extrahepatischer Metastasen sowie bei Lebermetastasen eines inkurablen Tumorleidens liegt eine palliative Situation vor. Der Lebensqualität kommt hierbei neben der Überlebenszeit eine essenzielle Bedeutung zu, die in bestimmten Situationen durch eine Operation entscheidend verbessert werden kann. Die heutige hepatobiliäre Chirurgie birgt jedoch trotz stetiger Verbesserung der perioperativen Medizin und der operativen Technik das Risiko signifikanter postoperativer Morbidität und Mortalität, weshalb das Vorgehen in der palliativen Situation sorgfältig abzuwägen ist. Neuroendokrine Lebermetastasen (NELM) stellen die Hauptindikation für die palliative Leberchirurgie dar. Die individuelle Palliativtherapie bei unheilbaren Lebertumoren sollte grundsätzlich die Wünsche des Patienten und eine psychoonkologische Behandlung einschließen.
