Zusammenfassung
Die Palliativmedizin hat sich in den letzten 50 Jahren rasant zu einem interdisziplinären Handlungsfeld entwickelt, in dem Chirurgen zu selten und oft erst sehr spät eingebunden werden, vielleicht aufgrund der Befürchtung, dies sei gleichbedeutend mit großen, belastenden Operationen. Diese Sichtweise greift aber zu kurz. Vielmehr können Chirurgen Palliativpatienten auf diverse Art unterstützen, wenn andere Behandlungsoptionen ausgeschöpft sind. Dies setzt allerdings eine intensive Auseinandersetzung mit der individuellen Erkrankungssituation voraus, die Chirurgen dazu herausfordert, sich zumindest vorübergehend von ihrer pragmatischen und zügigen Symptomkategorisierung und notwendigen Handlungen zu verabschieden. In der Verbindung aus feinsinnig-empathischer Situationsanalyse und pragmatischer Handlungsbereitschaft werden Chirurgen in der Palliativsituation als helfend und Leid lindernd wahrgenommen. Die einfache Übertragung von Denkprinzipien aus der kurativen Tumorchirurgie greift zu kurz. Die palliative Ausbildung kommender Chirurgengenerationen auszubauen muss ausgesprochenes Ziel sein.
