Zusammenfassung

Bei der Multimorbidität liegen mindestens zwei chronische Erkrankungen vor. Mittlerweile konnten typische Krankheitsgruppen identifiziert werden, die gehäuft zusammen auftreten: metabolische und kardiovaskuläre, neurologische und psychische Erkrankungen kombiniert mit Schmerzen oder somatoformen Störungen und muskuloskelettale Erkrankungen zusammen mit Übergewicht. Therapieentscheidungen basieren heutzutage zunehmend auf dem Prinzip der partizipativen Entscheidungsfindung. Problematisch ist die Umsetzung von Leitlinien für jede einzelne dominierende Erkrankung, da hierdurch Therapiekonflikte und Polypharmazie gefördert werden. Hilfreich für dieses Dilemma können neben didaktischen Krankheitsanalysen im Sinne von Einzelfallvignetten (N-of-One-Guideline) das Prinzip „Klug entscheiden“ (Choosing wisely) und selbstverständlich die stete Überprüfung von Diagnosen und Therapien im Behandlungsverlauf unter Berücksichtigung der Patientenpräferenzen sein.
