Zusammenfassung

Traumatisierung in der Kindheit führt zu verschiedensten schwerwiegenden Störungen in Kindheit, Jugend und Erwachsenenalter. Traumatisierung wird hier in einem weiten Sinne verstanden; z. B. Entwicklungs- und Bindungstraumatisierung, frühe Verluste, schwere Erkrankungen, Gewalt und Missbrauch. In diesem Kapitel beschreibe ich die Auswirkungen der Traumatisierung auf die Ich- und Selbstorganisation und befasse mich mit Behandlungsprinzipien. Als theoretische Grundlage stütze ich mich auf die Konzeption von Fonagy, Target et al. (2006), die Zusammenhänge von Traumatisierung in der Kindheit und gestörter Mentalisierungsentwicklung postulieren. Die Autoren legen dar, dass bedingt durch das Trauma die äußere Welt ständig psychisch bewacht werden muss und kein Raum bleibt für die Vorstellung einer damit verbundenen, aber dennoch getrennten inneren Welt. Entweder wird die innere Welt wie die äußere erlebt (Äquivalenzmodus), oder der Traumatisierte blendet die Wahrnehmung der äußeren Welt durch Dissoziation aus (Als-ob-Modus), um die Verbindung zwischen seinen inneren Zuständen und einer unerträglichen äußeren Realität zu durchtrennen. Neurobiologische Aspekte werden im Sinne des bio-psycho-sozialen Modells einbezogen.
