Zusammenfassung

Die Chirurgie ist die Standardbehandlung für die meisten Patienten mit lokalisiertem Weichgewebssarkom. Für Patienten mit resektabler Erkrankung ist die weite Resektion durch normales nicht-befallenes Gewebe das chirurgische Vorgehen der Wahl. Wo eine weite Exzision aufgrund anatomischer Hemmnisse nicht möglich ist, kann eine geplante marginale Resektion oder ein mikroskopisch positiver Rand gegen eine kritische Struktur plus Radiotherapie, bei mäßig und schlecht-differenzierten Tumoren, eine geeignete Maßnahme darstellen, eine Tumorkontrolle zu erreichen. Für Patienten mit grenzwertig resektablen Tumoren sollte eine präoperative Behandlung mit Chemotherapie und/oder Radiotherapie in Betracht gezogen werden, abhängig von der Histologie.
