Zusammenfassung

Hiatushernien werden in die Typ I–IV eingeteilt. Die Typen II–IV sind durch einen paraösophagealen Anteil charakterisiert und deutlich weniger verbreitet als die axiale Gleithernie (Typ I). Hiatushernien mit paraösophagealer Beteiligung sollten operiert werden, wenn sie symptomatisch sind, da das Risiko von Komplikationen besteht. Ziel der chirurgischen Therapie ist die dauerhafte Reposition von Bruchsack und Bruchinhalt sowie die Reparation des Hiatus. Für die meisten Hiatushernien ist der minimalinvasive, transabdominelle Zugang die bevorzugte Methode. Neben der Wahl des Zugangs müssen für eine optimale Hiatusreparation die Präparationsweise des Bruchsacks, die Art der Hiatoplastik, der Nutzen und das Risiko einer Netzverstärkung sowie die Notwendigkeit einer zusätzlichen Fundoplikatio als wichtige chirurgische Schritte betrachtet werden. Dieses Kapitel gibt zu diesen Punkten einen evidenzbasierten Überblick sowie Empfehlungen entsprechend der Literatur.
