Zusammenfassung
Die MILOS-Technik ist das erste minimalinvasive Operationsverfahren, das bei nahezu allen Bauchwand- und Narbenhernien eine präperitoneale bzw retromuskuläre Kunststoffnetzeinlage ermöglicht. Es handelt sich um ein Hybridverfahren, bei dem die extraperitoneale Präparation zunächst transhernial mit lichtarmierten laparoskopischen Instrumenten unter direkter Sicht oder endoskopisch assistiert erfolgt. Nach transhernialer Präparation eines mindestens 8 cm großen extraperitonealen Raums kann die Operation in Analogie zur TEP der Leistenhernie endoskopisch (EMILOS) fortgeführt werden. Eine prospektive Registerstudie unter Einschluss von über 600 MILOS-Narbenhernienoperationen zeigt im Vergleich mit der laparoskopischen IPOM-Operation und offenen Sublay-Operation signifikant weniger Komplikationen, chronische Schmerzen und Rezidive nach einem Jahr.
