Zusammenfassung
Die durch Neointimaproliferation verursachte Restenose nach Angioplastie lässt sich durch Beschichtung von Stents und Ballonkathetern mit Arzneimitteln hemmen. Bei der Entwicklung der beschichteten Stents wurden geeignete Arzneistoffe und Tiermodelle identifiziert. Ballons werden mit Paclitaxel beschichtet, das während der Balloninsufflation freigesetzt wird. Der Beschichtung werden Hilfsstoffe beigefügt, um Haftung, Freigabe vom Ballon und Wirkung zu verstärken. Das fast unlösliche Paclitaxel wird als Feststoff in der Arterienwand deponiert und bildet dort über Wochen bis Monate ein Reservoir. Mögliche Unverträglichkeitsreaktionen von Paclitaxel sind auch bei hoher Dosierung durch die geringe Löslichkeit begrenzt. Das Embolierisiko ist wesentlich geringer als Effekte durch Plaqueabrieb. Funktionelle Defizite in nachgeschaltetem Gewebe wurden auch bei hoher Dosierung nicht gefunden. Es bleiben die Vorteile der lokalen zielgerichteten Pharmakotherapie gegenüber der systemischen Behandlung, ohne dass ein Dauerimplantat erforderlich ist.
