Zusammenfassung
Verletzungen der oberen Extremität sind häufig und selten lebensbedrohlich. Die Erfassung der akuten Pathologien erfolgt nach dem etablierten ATLS-Schema. Es gilt, in der akuten Phase die Sekundärschäden z. B. durch Gefäßverletzungen (Ischämie) oder Logendruckerhöhung (Kompartmentsyndrom) zu vermeiden. Die Pathologien sind obligat frühzeitig zu erfassen und der adäquaten Therapie zuzuführen. Die Versorgungsstrategie der oberen Extremität richtet sich nach dem Damage-Control-Prinzip und muss sich hier in der notwendigen (lebenserhaltenden) Hierarchie eingliedern. Entscheidungen zur Replantation oder Amputation sind individuelle patientenbezogene Entscheidungen und müssen immer den Gesamtzustand des Patienten beachten. Es gilt «life before limb».
