Zusammenfassung
Der missbräuchliche oder abhängige Tabakkonsum liegt sowohl hinsichtlich der Prävalenz als auch der gesundheitlichen Bedeutung vor Alkohol und den illegalen Drogen. Bei etwa 50% der regelmäßigen Raucher kann eine Abhängigkeit diagnostiziert werden. Der Entstehung der Tabakabhängigkeit liegen psychologische und neurobiologische Mechanismen zugrunde: für die Aufrechterhaltung des Tabakkonsums sind einerseits die neurobiologischen Wirkungen von Nikotin verantwortlich, andererseits auch die intraindividuelle Funktionalität des Rauchens im Zusammenspiel mit psychosozialen Lebensbedingungen. Daher sind sowohl lerntheoretische Modelle als auch pharmakologische Ansätze für die Therapie bedeutsam. In den aktuellen S3-Behandlungsleitlinien werden neben allgemeinen Strategien zur Diagnostik und Frühintervention insbesondere verhaltenstherapeutische Verfahren im Einzel- oder Gruppensetting, medikamentöse Unterstützungen mit Nikotin, Bupropion oder Vareniclin sowie deren Kombination empfohlen.
