Zusammenfassung
Asthma ist eine variable und reversible Atemwegsobstruktion infolge Entzündung und Hyperreaktivität der Atemwege, deren genaue Ursache derzeit nicht bekannt ist. Umwelteinflüsse, Erbfaktoren, primäre Schädigungen, pathologische Immunreaktionen und eine gesteigerte Empfindlichkeit gegenüber unspezifischen Reizen sind beteiligte Faktoren. Beim akuten Anfall spielt v. a. der Bronchospasmus eine wichtige Rolle, während bei der Stunden, Tage oder Wochen anhaltenden Atemwegsobstruktion die Entzündungsreaktion im Vordergrund steht, wenngleich auch hier ein Spasmus beteiligt ist. Insgesamt überlagern sich aber diese pathogenetischen Mechanismen. Die Obstruktion der Atemwege führt zum Anstieg des Atemwegswiderstands mit Zunahme der Atemarbeit und bei entsprechender Ausprägung zu Störungen der Ventilation und des pulmonalen Gasaustausches.
