Zusammenfassung
Massenrupturen der Rotatorenmanschette weisen definitionsgemäß Sehnendefekte von über 5 cm Durchmesser auf und erstrecken sich über 2 und mehr Sehnen. Die Behandlung richtet sich nach Beschwerdesymptomatik und Funktionsausfall sowie nach klinisch und bildgebend prognostischen Faktoren wie einer ggf. bestehenden Muskelatrophie und -verfettung sowie einem Humeruskopfhochstand, der v. a. bei fortgeschrittener Infraspinatusbeteiligung beobachtet wird. Diese Veränderungen sind nach vorliegender Datenlage irreversibel. Beim älteren Patienten mit Defektarthropathie besteht die Indikation zu einer inversen Prothese oder ggf. zu schmerzorientierten palliativen, arthroskopisch durchzuführenden Operationen mit Tuberkuloplastik und Bizepssehnentenotomie. Beim jüngeren, aktiven Patienten ist für die Schmerzfreiheit und Wiederherstellung der Schultergelenkfunktion sowie zur Verhinderung einer Defektarthropathie eine Muskeltransferoperation indiziert. Die Technik des Latissimus-dorsi-Transfers wurde erstmals 1988 von Gerber vorgestellt und seither mehrfach modifiziert.
