Zusammenfassung
Schaftfreie Prothesen wurden erstmalig im Jahr 2004 implantiert. Derzeit bieten die meisten Hersteller schaftfreie Prothesen an. Das Prinzip beruht auf einer zementfreien metaphysären Verankerung. Dabei unterscheiden wir zwischen einer reinen metaphysären Press-fit-Verankerung und einer kombinierten metaphysären Press-fit- mit Zugschraubenverankerung über eine epiphysäre Abstützung durch den Kalottenträger. Als Vorteile beschreiben die Autoren eine intraoperative Zeitersparnis durch den schaftfreien Einsatz, einen geringeren Blutverlust, die Schonung des Humerusschaftes und ein geringeres Risiko für periprothetische Frakturen. Vor allem ist dieser Prothesentyp auch bei posttraumatischen Situationen mit bestehender Deformität einsetzbar. Auch im Falle eines notwendigen Revisionseingriffes stellt sich die Explantation der Prothese viel einfacher dar als der Ausbau der Stielprothese. Nach Explantation kann für die Reimplantation eine Schaftprothese mit einer Standardlänge verwendet werden.
