Zusammenfassung

Der Begriff Rektumprolaps beschreibt den vollständigen oder auch partiellen Vorfall des Rektums. Prinzipiell kann dies in verschiedenen Altersgruppen und bei beiden Geschlechtern auftreten, in der Majorität sind jedoch Frauen in einem Alter über 50 Jahren betroffen. Die Ursachen sind multifaktoriell und damit kann die Abklärung eine umfangreiche Diagnostik nach sich ziehen. Dabei sollten alle diagnostischen und therapeutischen Maßnahmen auf die individuelle Patientensituation abgestimmt sein. Je geringer der Leidensdruck, desto weniger Handlungsbedarf resultiert daraus. In seiner Komplettausprägung wird ein Rektumprolaps durch keine konservative Therapie zu behandeln sein, allerdings können allgemeine Maßnahmen die begleitende Symptomatik durchaus lindern. Insgesamt stehen mehrere, sehr unterschiedliche operative Konzepte zur Verfügung. Alle haben ihre Berechtigung, alle sind mit unterschiedlichen komplikativen Risiken und unterschiedlichen Erfolgsaussichten versehen. Jedoch sollte bei einem per se benignen Grundleiden nicht die Rezidivvermeidung, sondern die Komplikationsminimierung im Vordergrund der Behandlung stehen.
