Zusammenfassung
Komplexe und schwierige Aneurysmen sind definiert als Riesenaneurysmen (größer als 2 cm), fusiforme oder Mega-Dolicho-Aneurysmen sowie intrakavernös-perikavernöse Aneurysmen. Das operative Management dieser Aneurysmen bleibt eine große Herausforderung. Die meisten von ihnen können durch eine Kombination von Clipping und Mikroanastomosen-Backup-Bypass behandelt werden. Jedoch erfordern einige der sehr großen geschwungenen oder Riesenaneurysmen einen mikrochirurgischen Schädelbasis-High-Flow-Bypass, der als »Microsurgical Flow Diversion« bezeichnet wird. Dieses Kapitel fasst die persönliche operative Serie des Autors von 402 Fällen komplexer und schwieriger Aneurysmen zusammen. Verschiedene aktualisierte Schädelbasiszugänge und die Mikroanatomie werden ebenso wie verfeinerte Schädelbasis-Mikroinstrumente und spezielles Equipment im Detail beschrieben.
