Zusammenfassung
PIPAC ist ein neues regionales Verfahren zur Therapie der Peritonealkarzinose. Die Sicherheit und Effektivität dieses Verfahrens wurden in Fallbeobachtungen, retrospektiven Kohortenstudien und einer prospektiven Phase-II-Studie an Patientinnen mit Ovarialkarzinomrezidiv dokumentiert. Neben der direkten zytotoxischen Wirkung auf die Peritonealkarzinose bietet die PIPAC aufgrund ihrer positiven Effekte auf die Lebensqualität eine Alternative zur palliativen systemischen Chemotherapie. In Untersuchungen an unterschiedlichen Patientenkollektiven konnte gezeigt werden, dass es unter PIPAC zu einer Tumorregression bei gleichzeitiger Verbesserung gastrointestinaler Symptome wie Übelkeit, Erbrechen und Diarrhoe, einer Verbesserung der Gesamtlebensqualität sowie einer Verbesserung funktioneller Parameter wie Rollenfunktion, soziale Interaktion und physische/kognitive Leistungsfähigkeit kommt. Für Patienten mit einem langandauernden metastasierten Krankheitsverlauf und wiederholten systemischen Chemotherapien stellt die PIPAC eine Therapiealternative dar.
