Zusammenfassung
Durch die Etablierung der CRS mit HIPEC lässt sich bei einigen Malignomen mit peritonealen Manifestationen eine deutlich bessere Prognose erzielen. Wenn eine Peritonealkarzinose unerwartet intraoperativ gefunden wird, sollte daher ein Vorgehen gewählt werden, mit dem diese Chance gewahrt bleibt. Dafür wurde beim kolorektalen Karzinom als Primärtumor vor kurzem ein Algorithmus publiziert, der davon ausgeht, dass die definitive Operation im Sinne einer CRS und HIPEC gemeinsam in einem Eingriff durchgeführt werden sollte. Es wird daher empfohlen, die Primäroperation nach histologischer Sicherung und exakter Exploration zu beenden. Nur bei symptomatischem Primärtumor kann eine limitierte R0-Resektion oder die Anlage eines Stomas erfolgen. Die Indikation zur CRS mit HIPEC sollte dann entsprechend den definitiven Befunden interdisziplinär und unter Einbeziehung eines Zentrums für peritoneale Malignome diskutiert werden.
