Zusammenfassung
Die zytoreduktive Chirurgie (CRS) und die hypertherme intraperitoneale Chemotherapie (HIPEC) sind Teil der S3-Leitlinien kolorektaler Karzinome. Kritierien für die Durchführung einer erfolgreichen Operation (komplette Zytoreduktion, CCR 0) sind ein PCI unter 17 sowie das Fehlen eines diffusen Befalls von Dünndarm und Dünndarmmesenterium, Lymphknotenmetastasen, dem Nachweis einer geringen Differenzierung der Tumorzellen oder von Siegelringzellen in der Histologie sowie einer ausgedehnten hepatischen, extraperitonealen oder retroperitonealen Metastasierung. Dahingegen verschlechtern bis zu 3 resektable Lebermetastasen, weibliches Patientengeschlecht, die Lokalisation des Primarius, eine neoadjuvante Chemotherapie oder eine muzinöse Tumorkomponente das Outcome nicht. Die Indikation zur CRS und HIPEC sollte im Rahmen eines interdisziplinären Tumorboards gestellt werden. Bei einer sorgfältigen Patientenauswahl liegen Morbidität und Mortalität der CRS und HIPEC in einem akzeptablen Rahmen.
