Zusammenfassung
Schmerzen unter der Geburt werden individuell unterschiedlich wahrgenommen, oftmals aber als unerträglich beschrieben. Fragt man vor Anlage einer neuroaxialen Analgesie nach der Schmerzintensität, werden vielfach Werte auf der numerischen Ratingskala (von 0 bis 10) zwischen 8 und 10 angegeben. Geburtsschmerz imponiert somit vielfach stärker als ein Frakturschmerz. Zwar kommen komplementärmedizinische Interventionen, weil sie dem Wunsch der Hebammen oder auch der Schwangeren entsprechen, gerade in Deutschland nicht selten zum Einsatz, doch deren Anwendung ist weder gut belegt noch ausreichend gegen Geburtsschmerz wirksam. Insofern sollte alles unternommen werden, die Schwangere angstfrei – und dies bedeutet auch angstfrei im Hinblick auf Schmerzen – auf die Geburt vorzubereiten und durch die Geburt zu führen. Dies impliziert auch, dass tunlichst der Eindruck vermieden werden sollte, die Schwangere habe versagt, wenn eine rückenmarknahe Regionalanalgesie bzw. wirksame alternative Verfahren als gegenwärtiger Goldstandard in der geburtshilflichen Analgesie erforderlich waren.
