Zusammenfassung
Die fetale Magnetresonanztomographie (MRT) hat in den letzten Jahren erheblich an klinischer Bedeutung gewonnen. Sie ist in der pränatalen Diagnostik als zusätzliches, komplementäres Verfahren zur Standardmethode, dem Ultraschall, zu sehen. Die meisten fetalen MRT-Untersuchungen werden zur Abklärung unklarer ZNS-Befunde angefertigt, aber auch Fragestellungen zu Thorax, Abdomen oder Urogenitalsystem werden bearbeitet. Das Kapitel erläutert verschiedene Aspekte der fetalen MRT-Bildgebung, die von technischen und sicherheitsrelevanten bis hin zu den diagnostischen Möglichkeiten reichen. Bisher gibt es in der Literatur keine standardisierten Kriterien, wann ein ergänzendes fetales MRT sinnvoll ist. Zur Einordnung der Wertigkeit der fetalen MRT existieren einige Studien, die Ultraschall und MRT vergleichen. Diese Studien werden dargelegt und kritisch diskutiert. Einigkeit besteht, dass die diagnostische Wertigkeit der ergänzenden fetalen MRT in der fortgeschrittenen Schwangerschaft erhöht ist. Es muss immer daran erinnert werden, dass sowohl Ultraschall als auch MRT untersucherabhängige Verfahren sind. Wichtig sind vor allem die interdisziplinäre Kooperation und die gemeinsame Diagnosefindung anhand aller verfügbaren Befunde.
