Zusammenfassung

Die laparoskopische Heller-Myotomie (LHM), kombiniert mit einer Dor-Fundoplikatio, ist das Standardverfahren zur Erstlinientherapie der primär idiopathischen Achalasie, da Metaanalysen (Level 1A Evidenz) die Überlegenheit gegenüber der Ballondilatation zeigen. Als neues, rein endoskopisches Therapieverfahren wird die perorale endoskopische Myotomie (POEM) intensiv klinisch erprobt. Langzeitergebnisse und die Ergebnisse einer prospektiv randomisierten Multicenterstudie zum Vergleich mit der LHM stehen allerdings aus.
