Zusammenfassung

Im Verlauf einer nekrotisierenden Pankreatitis kommt es oft zu einer interventionspflichtigen Superinfektion der zuvor sterilen Nekroseareale. Die Nekrosen können auch dauerhaft als »walled-off pancreatic necrosis« (WOPN) bestehen bleiben. Bis vor wenigen Jahren erfolgte die Behandlung mittels offener Nekrosektomie mit einer hohen Morbidität und Mortalität. Seit einigen Jahren wird nach dem Step-up-Konzept zunächst interventionell drainiert und eine chirurgische Therapie nur falls notwendig durchgeführt. Diese erfolgt in der Regel als minimalinvasive transgastrale oder retroperitoneoskopische Nekrosektomie. Die offene Operation wird meist nur noch zur Beherrschung schwerwiegender Komplikationen (Blutung, Hohlorganperforation, Ischämie) eingesetzt.
