Zusammenfassung

Die laparoskopische Pankreaslinksresektion eignet sich als rein resektives Verfahren hervorragend für eine laparoskopische Operationstechnik. Die derzeitigen Studien zeigen Machbarkeit und Sicherheit der Operation auch hinsichtlich onkologischer Aspekte. Die wesentlichen Vorteile der laparoskopischen Operation liegen in der Rate des gesteigerten Milzerhaltes bei nichtonkologischen Operationen und dem deutlich verringerten Krankenhausaufenthalt. Die Fistelrate nach Pankreaslinksresektion ist weiterhin relativ hoch und unabhängig vom laparoskopischen oder offenen Vorgehen. Beschichtung des Staplers, langsame Kompression des Parenchyms und mittlere bis große Klammerhöhe konnten in nichtrandomisierten Studien die Pankreasfistelrate nach Pankreaslinksresektion reduzieren.
