Zusammenfassung
Jede Herzoperation erfordert die sorgfältige, lückenlose Überwachung der Herz-Kreislauf-Funktion. Zum Standard gehören die kontinuierliche EKG-Überwachung (Ableitung II und V5), die arterielle Kanülierung mit kontinuierlicher Blutdruckmessung, ein mehrlumiger zentraler Venenkatheter (Messung des zentralen Venendrucks und der zentralvenösen O2-Sättigung, Zufuhr kardiovaskulärer Medikamente), die transösophageale Echokardiographie (TEE), v. a. bei Herzklappenoperationen und allen minimal-invasiven Eingriffen ohne direkte Sicht des Herzens; mit der TEE kann außerdem die globale und regionale Myokardfunktion überwacht werden. Der Routineeinsatz des Pulmonaliskatheters ist dagegen nicht gerechtfertigt, da die meisten hämodynamischen Daten – weniger invasiv und gefahrlos – mit der Echokardiographie ermittelt werden können. Dies gilt auch für die HZV-Messung durch Pulskonturanalyse und ihre Varianten. Auch die Messung des linken Vorhofdrucks ist speziellen Indikationen vorbehalten.
