Zusammenfassung
Wichtigstes chirurgisches Behandlungsverfahren der Karotisstenose ist die Thrombendarteriektomie (TEA) mit oder ohne Patchplastik der A. carotis interna. Durch die Operation kann bei symptomatischen 70- bis 99- %igen Karotisstenosen (Stadium II) das Risiko des gleichseitigen Schlaganfalls hochsignifikant (um 16 %) und dauerhaft gesenkt werden. Die Operation kann – bei gleichwertigen Ergebnissen – in Standardallgemeinästhesie (angenehmer für den Patienten) oder in Regionalanästhesie (Plexus-cervicalis-Block) am wachen Patienten (bessere neurologische Beurteilbarkeit) vorgenommen werden. Der arterielle Blutdruck sollte während der Operation im Bereich der Ausgangswerte liegen und beim Abklemmen der A. carotis interna etwa 15–20 % darüber, jedoch <185/110 mm Hg betragen. Starke Blutdruckanstiege und Tachykardien müssen umgehend behandelt werden, besonders beim Koronarkranken. Hyper- und Hypoventilation während der Narkose sind zu vermeiden.
