Zusammenfassung

Eine Erweiterung der infrarenalen Aorta auf ≥3 cm wird als Bauchaortenaneurysma (BAA) bezeichnet. Spätestens ab 6 cm sollte die elektive (aber dringliche) operative Ausschaltung erfolgen. Der betroffene Aortenanteil wird – in TIVA oder eine balancierte Anästhesie – durch eine Dacron-Prothese überbrückt. Die postoperative Behandlung umfasst die Überwachung der Herz-Kreislauf-Funktion mit Vermeidung postoperativer Blutdruckanstiege, eine ausreichende Schmerztherapie und die regelmäßige Kontrolle der peripheren Pulse in den unteren Extremitäten. Die pAVK umfasst stenosierende oder okkludierende Prozesse der Extremitätengefäße, außerdem stenosierende Aortenprozesse. Ab Stadium IIb sind Maßnahmen zur Wiederherstellung der Strombahn indiziert. Der akute Gefäßverschluss, v. a. der embolisch bedingte, ist ein Notfall, der eine umgehende Intervention erfordert. Gefäßoperationen können in Allgemein- oder in Regionalanästhesie durchgeführt werden.
