Zusammenfassung
Die meisten herzchirurgischen Patienten stehen unter Erhaltungsdosen kardiovaskulärer Medikamente, hierunter v. a. β-Blocker, Antihypertensiva und Antiarrhythmika. Die Zufuhr dieser Medikamente wird in der Regel bis zum Operationstag fortgesetzt, um eine kardiovaskuläre Instabilität zu verhindern. Perioperativ treten sehr häufig Störungen der Herz-Kreislauf-Funktion auf, die akut mit Medikamenten behandelt werden müssen. Zu diesen Störungen gehören Blutdruckabfälle, Blutdruckanstiege, ein Low-output-Syndrom, Myokardischämien und Herzrhythmusstörungen. Die Behandlung erfolgt mit mit positiv-inotropen Substanzen (z. B. Adrenalin oder Dobutamin), Vasopressoren (z. B. Noradrenalin), Vasodilatatoren (z. B. Nitroglyzerin oder Kalziumantagonisten) und Antiarrhythmika (z. B. Propafenon oder Amiodaron). Die Zufuhr dieser Substanzen erfordert eine titrierte Anpassung der Dosis an den jeweiligen Bedarf des Patienten, kombiniert mit einer kontinuierlichen Überwachung der Herz-Kreislauf-Funktion.
