Zusammenfassung

Für chronisch kranke Jugendliche und deren Familien stellt der Übergang von der Kinder- und Jugendmedizin in die Erwachsenenmedizin eine kritische Phase dar. Zum einen wird die langjährige Arzt-Patienten-Beziehung gelöst und in neue Hände gegeben, zum anderen fällt diese Transition in eine sensible Lebensphase, in der junge Erwachsene auch ohne chronische Erkrankung eine Reihe von Entwicklungsaufgaben zu bewältigen haben. Das vorliegende Kapitel beschreibt nicht nur das Problem der Transition, sondern zeigt konkrete Lösungswege auf, die auf lange Sicht einen nationalen Standard darstellen sollten.
