Zusammenfassung
Hat ein Nierenpatient ein hohes Risiko für eine chronische Dialysebehandlung, sollten rechtzeitig am nichtdominanten Arm die Venen geschont („safe the veins“) und trainiert und der Patient dem Shuntchirurgen vorgestellt werden, der zur OP-Planung duplexsonographisch die arterielle Versorgung und den venösen Abstrom darstellt. Die Duplexsonographie ist das Mittel der Wahl zur morphologischen und funktionellen prä-, intra- und postoperativen Darstellung der Gefäße. Zur Vermeidung eines frühen Shuntversagens sollten postoperative duplexsonographische Untersuchungen erfolgen. Im Fall einer Shuntdysfunktion und duplexsonographisch nicht ausreichender Darstellbarkeit der zentralen Gefäße kommen alternativ angiographische Untersuchungsmethoden zur Anwendung, vorzugsweise die CO2-Angiographie bei Patienten mit erhaltener Restnierenfunktion.
