Grundlagen der Therapie zerebraler arteriovenöser Malformationen

Zusammenfassung

Zerebrale arteriovenöse Malformationen (AVM) sind komplexe Läsionen. Ein gut ausgeprägtes Verständnis der Gefäßpathologie und fundierte Erfahrungen mit der Behandlung sind Voraussetzungen, um adäquate Behandlungsentscheidungen treffen zu können. Es liegt eine robuste Datenlage zum natürlichen Verlauf vor. In diesem Kapitel werden wesentliche Daten zu natürlichem Verlauf, Biologie, Blutungsverhalten und therapeutischen Verfahren erläutert. Die wesentliche und aktuelle Datenlage wird dargestellt. Im Zusammenhang wird erläutert, welche AVM behandelt werden sollte. So wird auch dargelegt, dass eine differenzierte Beurteilung notwendig ist, um diejenigen Patienten zu identifizieren, die klar von einer Behandlung ihrer AVM profitieren. Die mikrochirurgischen Prinzipien der Entfernung werden detailliert beschrieben, und Hinweise auf Zugänge zu den einzelnen Lokalisationen aufgelistet.
