Zusammenfassung
Die beiden charakteristischen Lähmungsbilder der angeborenen Gelenksteife (auch Arthrogrypose oder Amyoplasie genannt) und der meist erworbenen Spastik gehen mit typischen klinischen Erscheinungsbildern an der oberen Extremität einher. Patienten mit einer Arthrogrypose verfügen über eine normale Sensibilität und Propriozeption. Bei der spastischen Lähmung findet sich dagegen ein Defekt in der zentralen Bewegungsplanung und ihrer peripheren Umsetzung. Patienten mit Arthrogrypose können ihre teilweise extrem bewegungseingeschränkte und kraftlose Extremität optimal nutzen, während Patienten mit einer Spastik abhängig von ihrer zentralen Schädigung eine vollständige Funktionslosigkeit zeigen können. Deshalb liegen die therapeutischen Konsequenzen bei der Arthrogrypose in einer bestmöglichen Erhaltung bzw. einem Ausbau der Beweglichkeit und der Kraft, während bei spastisch Gelähmten die Prävention von Kontrakturen und die Übungsbehandlung im Vordergrund stehen.
