Zusammenfassung
Beschrieben werden bewährte MCSS (Mechanical Circulatory Support Systems) mit Betonung auf heute verfügbare Systeme. Basierend auf den Erfahrungen des Deutschen Herzzentrum Berlin (DHZB), der Institution mit dem weltweit größten Systemspektrum und der höchsten Zahl von Patienten mit mittel- und langfristigen MCSS, werden Indikationen und spezielle Begleittherapien vorgestellt. Waren es zunächst in den 1980er-Jahren pulsatile pneumatische extrakorporale Systeme, gefolgt von elektromechanischen pulsatilen implantierbaren Ventricular Assist Devices der ersten Generation, so beherrschen heute die nichtpulsatilen implantierbaren Systeme der zweiten und dritten Generation den Markt für mechanische Herzunterstützung. Die Einsatzzahlen der pulsatilen Verdrängerpumpen gingen zugunsten der Systeme mit kontinuierlichem Fluss rapide zurück. Es scheint sicher zu sein, dass die Zukunft der mechanischen Herzunterstützung den Rotationsblutpumpen gehört. Die Systeme sind geräuschlos, werden weiter verkleinert, immer weniger invasiv implantierbar sein, neue fluiddynamische Designs und optimierte Blutkontaktoberflächen werden die Biokompatibilität noch weiter verbessern. Der heutige Stand der Technik und der medizinischen Erfahrungen erlaubt den Einsatz der mechanischen Herzunterstützungssysteme bei einer Vielzahl von Indikationen bis hin zum permanenten Ersatz der Herzfunktion mit vertretbaren Risiken und Einschränkungen der Lebensqualität.
