Hochmaligne Non-Hodgkin Lymphome beim alten und geriatrischen Patienten

Zusammenfassung

Das hochmaligne Non-Hodgkin Lymphom ist eine aggressiv verlaufende Erkrankung, die überwiegend aus B-Lymphozyten hervorgeht. Grundsätzlich ist das aggressive Non-Hodgkin Lymphom eine Erkrankung, die im höheren Lebensalter auftritt. Das mediane Alter zum Zeitpunkt der Diagnosestellung beträgt ca. 65 Jahre. Therapie der Wahl ist die Systemtherapie bestehend aus dem monoklonalen Antikörper Rituximab und der Kombinationschemotherapie nach dem CHOP Protokoll. Begleitet wird die Therapie von einer anti-infektiösen Supportivtherapie. Die systemische, chemotherapeutische Behandlung erfolgt sowohl bei alten als auch sehr alten Patienten mit einem kurativen Behandlungsziel. Allerdings muss insbesondere bei sehr alten Patienten >80 Jahre die Therapie dosisreduziert nach dem R-mini CHOP verabreicht werden. Neben dem Stadium der Erkrankung wird die Prognose der Erkrankung entscheidend vom Allgemeinzustand bestimmt.
